Another application of forms4health is forms4patients; a patient facing tool, allowing clinicians to efficiently gather vital patient data whilst improving the patient experience.
Significant clinical time, particularly in consultations, is spent navigating and completing exhaustive forms. Similarly, patients are often required to fill in large, daunting forms, where perhaps only a few sections are actually relevant. These forms are often completed in fairly pressured conditions, so the patient may not have time to consider their answers carefully or check vital details (such as the names of existing medications or the date of a previous procedure.)
With forms4patients, forms can be designed for patients to complete, either at their own convenience, or in a clinical setting with assistance from staff. The forms are quick, easy, and clear to complete, and can provide valuable data to inform the patient’s care, such as pre-surgical assessment forms.
As with all resources created through forms4health, the smart forms can be designed with inbuilt branching logic, so patients are only shown the questions relevant to them. Direct integration with the patient record means data from the EHR can pre-populate fields in the forms, whilst also allowing the patient’s responses to be written back into the record. This saves time for both the patient and clinician receiving the data, and reduces the risk of errors.
A simple online process between the organisation and the patient, including authentification stages, keeps the experience as easy and relevant for the patient as possible, whilst adhering to all IG standards.
