A pre-determined form or assessment can be sent to a specific patient. This can happen prior to an appointment, for the patient to complete at their own convenience, or the patient could be given the assessment to complete on arrival on a mobile device, before seeing the clinician. The smart form would be designed to only show the patient relevant questions, and, once completed, would be sent to a specified email in the requested format.
